Very good list, but imho it should include Zeppelin's "Houses of the Holy" (child pr0n, omg!), and Jane's Addiction's "Nothing's Shocking".

I recall a pretty big outcry at some covers around the time of 9/11. Dream Theater released a live cd with a scene very reminiscent of the burning World Trade Center. To be fair the disc was released slightly before 9/11, coincidence I guess. After 9/11 they were selling em in stores with a sticker over top of the artwork, on purpose.

There was a hip hop album with something similar too if I recall.
